This Volunteer Liability Waiver and Release Agreement ("Agreement") is entered into between the undersigned volunteer ("Volunteer") and OneAmerica Votes ("Organization") as of the timestamped date of this submission.
1. Voluntary Participation
Volunteer agrees to participate in canvassing on a voluntary basis, without expectation of compensation, wages, or future employment.
2. Assumption of Risk
Volunteer understands that canvassing may involve certain inherent risks, including but not limited to:
- Walking or traveling in unfamiliar neighborhoods
- Interacting with members of the public
- Exposure to adverse weather conditions
- Risk of injury from falls, accidents, assault and battery, or encounters with animals
- Allergic reactions, including reactions to plants, pesticides, herbicides, fertilizers, food, insect bites, bee stings, dust, pollen, plastics, and soils
- Property damage or loss, including personal property that you bring to a canvassing event that could be lost, stolen, or damaged because of conditions from canvassing or the actions of individuals arising from the canvassing event
Volunteer assumes full responsibility for any personal injury, illness, damage, or loss that may result from their participation, whether caused by negligence or otherwise.
3. Release and Waiver
Volunteer hereby generally and completely releases Organization, its successors, current and former officers, directors, employees, agents, affiliates, related entities, sponsors, or other volunteers from any and all liability, claims, obligations, causes of action, and demands of whatever kind of nature, either in law or in equity, whether known or unknown, which arise or may hereafter arise from relating in any way to participation in services Volunteer provides to Organization
Volunteer also understands that Organization does not assume any responsibility for or obligation to provide financial assistance or other assistance, including but not limited to medical, health, or disability insurance in the event of injury or illness, or to compensate for lost property or damage.
Volunteer hereby releases and forever discharges Organization from any claim whatsoever which arises or may hereafter arise on account of any first aid, treatment, or service rendered in connection with the Volunteer’s Activities with Organization.
4. Conduct and Compliance
Volunteer agrees to:
- Abide by all applicable local, state, and federal laws
- Conduct themselves respectfully and lawfully
- Follow all training, instructions, and safety protocols provided by the Organization
The Organization reserves the right to revoke volunteer privileges at any time.
5. Minors
If the Volunteer is under 18 years of age, this form must be signed by a parent or legal guardian.
6. Criminal Background Exclusion
We respectfully decline the volunteering services of any person who has ever been convicted or accused of sex abuse or child molestation crimes of any kind at any time in the past; any such conviction or accusation means they cannot engage with canvassing with Organization. If you have ever been convicted or accused of sex abuse or child molestation, please do not volunteer to canvass with OneAmerica Votes.
☐ Volunteer affirms they have never been convicted or accused of sex abuse or child molestation crimes of any kind at any time in the past;
☐ Volunteer understands that any conviction or accusation of sex abuse or child molestation crimes means they cannot volunteer to canvass with OneAmerica.
7. Insurance
The Volunteer understands that, except as otherwise agreed to by Organization in writing, Organization does not carry or maintain health, medical, or disability insurance for any Volunteer. Volunteer affirms they are expected and encouraged to obtain their own medical or health insurance coverage. Volunteer expressly waives any such claim for compensation or liability on the part of Organization beyond what may be offered freely by Organization in the event of injury or medical expenses incurred by Volunteer.
We expect volunteers who are using their vehicles to attend our events to have their own auto insurance. We waive liability on vehicular-related incidents before, during, and after the canvassing event.
8. Photo Release (optional)
I hereby grant permission to the rights of my image, likeness and sound of my voice as recorded on audio or video tape without payment or any other consideration. I understand that my image may be edited, copied, exhibited, published or distributed and waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my image or recording.
By signing this release I understand this permission signifies that photographic or video recordings of me may be electronically displayed via the Internet or in an educational setting.
There is no time limit on the validity of this release nor is there any geographic limitation on where these materials may be distributed.
This release applies to photographic, audio or video recordings collected by OneAmerica Votes.
If you do not grant permission for photo release, please indicate that as you fill out the form.

By signing this form I acknowledge that I am 18 or over have completely read and fully understand the above release and agree to be bound thereby. I hereby release any and all claims against any person or organization utilizing this material for educational purposes.
I HAVE READ THIS WAIVER AND RELEASE AGREEMENT AND FULLY UNDERSTAND ITS TERMS. I UNDERSTAND THAT I AM GIVING UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY.
This form is in effect for one year from the signing date. This Release and Waiver of Liability (the “Release”) executed on the time stamped day of this form by (the “Volunteer”) in favor of OneAmerica Votes, their directors, officers, employees, and agents.
